Proof of residence (Meldebescheinigung)
Birth Certificate (original or notarized copy)
Affidavit, stating that you are single and have never been married before. (Aufenthaltsbescheinigung zur Vorlage beim Standesamt)

All documents must be either in German, English or Danish language and the Affidavit must be issued not earlier than 3 month before the wedding date.
