In order to succeed with your request we would like you to do the following:

1. Fill out the Online Application and submit it to us
2. Then you open the Marriage Declaration PDF and
   - fill it
   - print it
   - sign it (both of you)
   - send it to us per post mail

Each couple has to fill only one form. You'll find our address here.
